The art of oncology: when the tumor is not the target. Don't mention it.

8208 Background: Because cancer patients and healthcare professionals have been shown to differ in their beliefs about what information is appropriate for cancer patients, it is necessary to obtain patient perspective on new sources of information. The purpose of the study was to identify patient opinion on the emotional impact, clinical impact, and appropriateness of sharing with cancer patients an anthology emphasizing end-of-life care. METHODS In this qualitative research study, 22 patients with advanced-stage cancer and 6 patient advocates received a copy of an anthology entitled, "The Art of Oncology: When the Tumor is Not the Target," and then completed an individual semi-structured interview assessing the emotional impact and clinical impact (e.g., patient-physician communication) of the anthology. Participants also made recommendations regarding sharing the anthology with other cancer patients. Interview transcripts were coded using methods of content analysis, facilitated through use of N6 qualitative data analysis program. RESULTS Overall, participants approved of sharing the anthology with select patients, especially those who are active in their care, accepting of their cancer status, and emotionally ready to read about end-of-life issues. For these patients, participants described the anthology as having potential for positive impact on patient-physician communication, patient-physician relationship (e.g., increased empathy for oncologists), patient consideration of treatment options, and patient preparations for end-of-life care. Nonetheless, the anthology was considered "depressing" and "lacking in hope" by several participants. Men were more likely than women to decline interview and/or immediately discontinue reading the anthology after experiencing a strong negative emotional response. CONCLUSIONS This anthology appears to be a valuable resource for some, but certainly not all, patients with advanced-stage cancer. If it is made available for such patients, it is recommended that appropriate warnings, regarding the content, be shared with potential recipients. No significant financial relationships to disclose.
